1. Met or talked with a Charter School Administrator to see if Endeavor Charter School is right for your child.

2. Completed the online Student Registration

3. Birth Certificate

4. Immunization records and TDAP Booster (or signed waiver-only Medical exemptions are accepted as of 7/1/16). View our Student Health and Immunization policy below.

5. Proof of TB test (or signed waiver-now requires health care provider signature)

6. Proof of residence (copy of utility bill, etc.)

7. Drop Grades and/or Copy of the student's transcript
